The Flip effect of "Worm Millidith" is a Trigger Effect that activates whenever it is Flipped face-up. Including during the Damage Step. It targets 1 Face-up monster in the opponent's Monster Zone.: Worm Millidith: Effect Monster > Worm Millidith

If "Worm Millidith" is flipped face-up as a result of being attacked and is destroyed by that battle, it cannot be equipped to a target by its effect.

The effect that inflicts damage to the controller of the equipped monster during each Standby Phase is only activated while "Worm Millidith" is treated as an Equip Card by its own effect. The effect starts a chain. The effect activates once per turn, during either player's Standby Phase.
