This monster is based off the Dungeons & Dragons representation of a Mesopotamian goddess, Tiamat. Dungeons and Dragons depicts Tiamat as being a five-headed goddess of evil dragons, with each head colored one of the primary colors of chromatic dragons - blue, green, red, black, and white.
Accordingly, Blue-Eyes White Dragon strongly resembles Bahamut, Tiamat's rival and god of good dragons. This is fitting, as Blue-Eyes is a LIGHT monster, one of two Attributes that can destroy Five-Headed Dragon in battle.
Its upgrade, Dragon Master Knight was also used to combat Five-Headed Dragon in the anime and is one of the few cards in the real game able to destroy Five-Headed Dragon by battle.
This monster has 5 heads: a center silver EARTH head, a mechanized WIND head, a black DARK head, a FIRE head, and a blue WATER head. There is no LIGHT based head, referencing its effect of only being able to be destroyed by LIGHT (and DIVINE) monsters.
This monster appears in the artwork of Paradox Fusion.
Four of this monster's heads resemble Serpent Night Dragon, Darkfire Dragon, Metal Dragon, and Aqua Dragon.
Coincidentally, Serpent Night Dragon and Aqua Dragon are two of the five monsters that merged to form this monster in Yu-Gi-Oh! Capsule Monsters. The other three are Meteor Dragon, Luster Dragon #2 and Tyrant Dragon.
This card has a Spellcaster-Type counterpart in Quintet Magician, as both cards are DARK Level 12 Fusion Monsters whose Fusion Materials are 5 monsters with the same Type.
This card has a retrained Link counterpart in Five-Headed Link Dragon.

TCG Rulings
Miracle Synchro Fusion: You cannot Summon "Five-Headed Dragon" using "Miracle Synchro Fusion" because it does not name a Synchro Monster as a Fusion Material Monster.
OCG Rulings
"Revoke Fusion" cannot Special Summon a Fusion Monster unless it is specifically named on the Fusion Monster. For example, you cannot send "Five-Headed Dragon" to the Graveyard to Special Summon a Dragon-Type monster.: Can "Five-Headed Dragon", or a Fusion Monster which does not state the names of its Fusion Material Monsters, be sent to the Graveyard for "Revoke Fusion"?
To activate the effect of "Elemental HERO Prisma", you can only send from the Deck to the Graveyard a monster whose card name is specifically stated as a Fusion Material Monster on the Fusion Monster. Thus, you cannot reveal "Five-Headed Dragon" and send any Dragon-Type monster to the Graveyard to activate the effect.: Can "Five-Headed Dragon" be chosen to be shown to the opponent as an activation cost for "Elemental HERO Prisma"?
Miracle Synchro Fusion: "Miracle Synchro Fusion" cannot Fusion Summon a Fusion Monster unless that Fusion Monster lists a Synchro Monster as a Fusion Material. Thus, you cannot use "Miracle Synchro Fusion" to Fusion Summon "Five-Headed Dragon" nor "Elemental HERO Absolute Zero", even if you are using a Synchro Monster as a Fusion Material. "Miracle Synchro Fusion" can only Fusion Summon Fusion Monsters such as "Dragon Knight Draco-Equiste" or "Naturia Exterio
Revoke Fusion: Fusion Monsters that do not specify any Fusion Material Monsters by name ("Five-Headed Dragon", etc.) cannot be selected with "Revoke Fusion".
Five-Headed Dragon" protects itself from being destroyed in battle against monsters with the specified Attributes, but it does nothing to protect the Life Points of the player controlling it or their opponent. Battle Damage is still inflicted to players.: Five-Headed Dragon
